For newly enrolling families–Please tell us a bit about your family, and your child’s special qualities.
If you wish to authorize someone other than yourself to pick up your child.
To be completed by you (upper portion) and your physician, based on most recent well child exam. Also obtain a copy of your child’s updated immunization record.
Be sure to sign and date both Page 1 and 2
